Frequently Asked Questions for Patients

Once your test results are provided to your physician, the Granger Genetics Patient Financial Services will submit a claim to any insurance company or other organization (“Payer”) who may be responsible for the cost of the tests. Generally, the Payer will provide you with an Explanation of Benefits (EOB) that indicates whether it made payment on the claim and the extent of your out-of-pocket liability. The EOB is not a bill from Granger Genetics. If you have questions about your EOB, you should call the Payer. Granger Genetics Patient Billing may also be able to explain the EOB to you. Granger Genetics works tirelessly on behalf of individuals to whom it has provided testing. If Granger Genetics believes that the Payer did not pay for the testing as it should have, it may file an appeal in accordance with the Payer’s policies and procedures.

After the process of billing and receiving payment from the Payer is complete, including resolution of any appeals, you may receive a billing statement from Granger Genetics Patient Billing. If you do not owe a balance, you will not receive a bill from Granger Genetics. Most payers (other than Medicare) do require some patient payment which may include deductible, co-insurance or co-pay amounts and Granger Genetics reserves the right to employ a third-party revenue recovery agency for delinquent accounts. The amount of the patient responsibility will depend on your Payer arrangements and the status of Granger Genetics with the Payer. Granger Genetics also offers payment plans and quick pay discounts. If the patient is in financial need, Granger offers a number of financial tools to help make testing possible for those who qualify.

The patient is responsible for all cost sharing amounts as assigned by the insurance company. Granger Genetics bills patients based on their explanation of benefits from the insurance company. If you have any questions on your deductible, co-insurance, or co-pay please contact your insurance company for an explanation. Granger Genetics reserves the right to employ a third-party revenue recovery agency for delinquent accounts. GRANGER GENETICS DOES NOT DETERMINE COST SHARING AMOUNTS.
If you have any questions about what your insurance plan will cover please contact your insurance company directly prior to testing. Granger Genetics does NOT know each patient’s individual coverage and benefits and cannot provide an exact amount of cost sharing owed by each individual patient. It is not until the claim has been finalized and an explanation of benefits has been received that Granger Genetics can determine a patient’s cost sharing.
Granger Genetics performs testing based on a signed requisition from a referring provider. Granger Genetics does not determine where your doctor sends your specimen. Please contact your doctor.
